RESOLUTION OF THE CITY OF DANIA, FLORIDA, APPROVING
UTILITY RELOCATION AGREEMENT WITH THE STATE OF
FLORIDA, DEPARTMENT OF TRANSPORTATION STATE
PROJECT NO. 86070-6487, ROAD NO. 9(t-95) & 848 (STtRLtNG
ROAD); AND PROVIDING FOR AN EFFECTIVE DATE.

BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY OF DANIA,
FLORIDA:
Section 1. That that certain Utility Relocation Agreement among the City of
Dania and State of Florida, Department of Transportation covering State Project No.
86070-6487 relating to the construction, reconstruction or othenruise change to a portion
of the State Highway System Road No. 9(l-95) & 848 (Stirling Road), from east of S.W.
20th Avenue to S.W. 18th Avenue. A copy of the Utility Relocation Agreement is
attached hereto and made a part hereof as Exhibit "A", be and the same is hereby
approved, and the appropriate city officials are hereby directed to execute same.
Section 2. That this resolution shall be in force and take effect immediately
upon its passage and adoption.
PASSED and ADOPTED this 28th day of November, 1995.

Dear Mr. Allen:
Enclosed please find the following plans and Relocation forms to finalize the utility relocations
for this Florida Department of Transportation project.
1. One (1) full set of 907o plans to update your file.
One ( 1) partial set of 90Vo plans to be color coded and retumed with the Utility
Relocation Schedule. The color-coded plans shall show the final disposition of your
facilities as follows: GREEN to indicate utilities to remain, RED to indicate the utility
is being removed or abandoned, and BBOUII! to indicate proposed new construction.
Form 710-01G172 (418188) Utility Relocation Schedule. Please complete, make four
copies, then execute all four with original signatures and return all four to this office with
the color coded plans. It is imperative that all information contained in this form be
presented as clear and concise as possible, and that the form be filled in completely using
a typewriter. This form becomes important in the project and construction scheduling and
may be included in the contract bid package.
Form 710-010-24 (07195) Utility Relocation Agre€ment (at Department expense). Please
complete, make four copies, then execute all four with original signatures and return all
four to this office.
Form 710-010-13 (8/95) City Resolution, Utility Relocation Agreement. Please complete
the resolution, or provide a certified copy of the City's own resolution form (also
acceptable).

necessary relocations as described in ttre Utility Relocation Schedule. Your efforts to expedite
this submittal is greatly appreciated for the success of this project.
