A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF DANIA
BEACH, FLORIDA, AUTHORIZING THE PROPER CITY OFFICIALS TO
APPROVE CREDIT CHANGE ORDER NO. 5 IN THE AMOUNT OF ELEVEN
THOUSAND SIX HUNDRED SIX DOLLARS AND FIVE CENTS ($11,606.05)
TO SAGARIS CORP. FOR THE DANIA BEACH CITYWIDE PEDESTRIAN
SAFETY (CRA MULTIMODAL IMPROVEMENTS) PROJECT; PROVIDING
FOR CONFLICTS; FURTHER, PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, the Dania Beach Citywide Pedestrian Safety Project (CRA Multimodal
Improvements) was planned, designed, and constructed for the safety and beautification of the
Byrd Point Neighborhood; and
WHEREAS, the City Commission awarded a bid to Sagaris Corp. for Four Hundred
Fifty Nine Thousand One Hundred Ninety Nine Dollars and Forty Cents ($459,199.40) on July
30, 2019, by Resolution 2019-083; and
WHEREAS, on May 26, 2020, the City Commission approved Change Order Number
One (1) in the amount of Fifty Nine Thousand Four Hundred Fifty Nine Dollars and Sixty Nine
Cents ($59,459.69), by Resolution 2020-046 for additional safety enhancements and adjustments
to field conditions; and
WHEREAS, on June 23, 2020, the City Commission approved Change Order Number
Two (2) in the amount of Sixty Four Thousand Six Hundred Seventy Eight Dollars and Thirty
Nine Cents ($64,678.39), by Resolution 2020-058 for additional materials to meet field
conditions; and
WHEREAS, on May 26, 2020, the City Commission approved Change Order Number
Three (3) in the amount of Three Thousand Five Hundred Dollars ($3,500.00), by Resolution
2020-046 for required quality control asphalt testing; and
WHEREAS, on May 26, 2020, the City Commission approved Change Order Number
Four (4) in the amount of Two Thousand Ninety Six Dollars and Forty Cents ($2,096.40), by
Resolution 2020-046, for temporary roadway markings; and
WHEREAS, Change Order Number Five (5), a credit change order, in the amount of
Eleven Thousand Six Hundred Six Dollars and Five Cents ($11,606.05) is required to decrease
the contract amount to Sagaris Corp. for material not required and work deleted from the
Agreement;
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF
THE CITY OF DANIA BEACH, FLORIDA:
Section 1. That the foregoing "Whereas" clauses are ratified and confirmed as being
true and correct, and they are made a specific part of and incorporated into this Resolution by
this reference.
Section 2. That the City Commission approves credit Change Order Number Five
(5) for the Dania Beach Citywide Pedestrian Safety (CRA Multimodal Improvements) Project
with Sagaris Corp., in the amount of Eleven Thousand Six Hundred Six Dollars and Five Cents
($11,606.05), a copy of which is attached as Exhibit "A", and authorizes and directs the City
Manager to execute it.
Section 3. That all resolutions or parts of Resolutions in conflict with any of the
provisions of this Resolution are repealed to the extent of such conflict.
Section 4. That this Resolution shall become effective immediately upon its passage
and adoption.

The following modifications to the CONTRACT are hereby because of The City of Dania Beach directed the Contractor to exclude
and give credit for scope of work on milling, enhanced crosswalk and thermoplastic white arrows of a raised intersection. The
location is SW 8th Ave, south of Dania Beach Blvd and NW 10th CT, north of Dania Beach Blvd.

Item No. 01 Milling Existing Asphalt Pavement 3-3/4 in.
The contractor excluded all labor, equipment and material to construct aproximately additional 24.89 SF of Milling Existing
Asphalt.
Item No. 02 Single Post Sign, F&I Ground Mount up to 12 SF
The contractor excluded all labor, equipment and material to F&I 4 Single Post Signs.
Item No. 03 Thermoplastic Standard White Arrow
The contractor excluded all labor, equipment and material for thermoplastic arrow 8 ea.
Item No. 04 Enhanced Crosswalks
The contractor excluded all labor, equipment and material to construct the enhance crosswalks identified on this intersection.
Item No. 05 Temporary Striping - CO # 4 - No. 3
The contractor excluded all labor, equipment and material to construct the placement of thermoplastic road marking.

The Dania Beach Housing Authority developed two properties. Site and right-of-way elevations raised after the Project's construction plans were developed. The
road between the two properties is at a lower elevation. Dania Beach Housing Authority will close on property on south side of W. Dania Beach Blvd and will develop
site. Prior to development, Housing Authority is required to raise road to level of existing level on either side. The City of Dania Beach decided against installing dec-
orative crosswalks in two adjacent locations because they will be removed to fix the road. The road needs to be engineered for drainage.

Pursuant to the field meeting held on 02/04 (following progress meeting), it was agreed to omit the
following work from plan sheet C 1.1
• Milling & Resurfacing on SW 8th Ave, south of Dania Beach Blvd
• Milling & Resurfacing on NW 10th CT,'north of Dania Beach Blvd
• Raised Intersection on Dania Beach Blvd & NW 10th Ct
Work to be omitted due to due construction & partial paving (by others) in the area.
Please provide concurrence.
