Home My WebLink AboutR-2000-184 RESOLUTION NO.-2000-184 A RESOLUTION OF THE CITY OF DANIA BEACH, FLORIDA AUTHORIZING THE CITY MANAGER TO PURCHASE ONE LIFEPAK 12 DEFIBRILLATOR/MONITOR FOR THE FIRE RESCUE DEPARTMENT AT A COST OF $17,565.00; PROVIDING THAT THE LIFEPAK BE PURCHASED FROM MEDTRONIC/PHYSIO-CONTROL, SOLE SOURCE PROVIDER, WITHOUT COMPETITIVE BIDDING AND WITHOUT ADVERTISEMENT FOR BIDS; PROVIDING FOR CONFLICTS; FURTHER, PROVIDING FOR AN EFFECTIVE DATE. WHEREAS, the Charter of the City of Dania Beach, Part III, Article 3, Section 4, Subsection (J), the city commission may, during unusual conditions or emergencies, by resolution, authorize the purchase by the city manager of designated supplies, services, equipment and materials in amounts in excess of Fifteen Thousand Dollars ($15,000.00) without competitive bids and without advertising for bids; and WHEREAS, the city manager has determined that it is necessary to purchase one Lifepak 12 defibrillator/monitor for use by the Fire Rescue Department from Medtronic/Physio-Control, 11811 Willows Road NE, P.O. Box 97006, Redmond, WA 98073-9706, the sole source provider of the equipment; and WHEREAS, the city manager has determined that such purchase can be made at the least cost to the city by purchasing said equipment from Medtronic/Phsio-Control, sole source provider, in the amount of $22, 565.00 with a $5,000.00 credit for a trade-in of one LifePack 10, Serial Number 003244, for a total expenditure to the City of $17,565.00; and NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY OF DANIA BEACH, FLORIDA: Section 1. That the city manager be and he is hereby authorized to pay from the Fire Department Budget the sum of Seventeen Thousand Five Hundred Sixty-Five and 1 RESOLUTION NO.2000-184 00/100 dollars ($17,565.00) to Medtronic/Physio-Control for one Lifepak 12 Defibrillator/Monitor. Brown, Deputy Fire Chief RE: Proposal for purchase of LifePack 12 DATE: November 6, 2000 Attached is information relating to the Lifepack 12 monitor/defibrillator. Our current budget has funds which have been designated for this purchase and a portion of the purchase price has been defrayed by Medtronic/Physio-Control allowing us a "trade-in" credit for our old LifePack 10, Serial Number 003244, which no longer fits our needs. You will please note that even though this quote appears to be firm only through October 27, 2000, that date has been extended through the date of the next commission meeting (November 14, 2000) to allow for approval of this purchase by the city commission. Accordingly, I am requesting that the city commission approve purchase of this important piece of equipment in the amount of $17,565.00 from Medtronic/Physio-Control, a sole source provider, pursuant to "Quotation # KJB5862486-B and "sole source letter", both of which have been furnished by Medronic/Physio-Control.
