AUTHORIZING THE APPROPRIATE CITY OFFICIALS
TO EXECUTE THE SERVICE AGREEMENT BETWEEN

SERVICE AGREEMENT
By and Between
MARY C . McCAUGHEY-BLASI CITY OF DANIA

( "Blasi ") ( "City" )
A. SCOPE OF SERVICES:
Blasi will provide the following services to the City
relating to the function of Finance Administrator:
1 . Blasi will be available during established office
hours, as are mutually agreed upon between Blasi and
the City Clerk/Auditor with the approval of the City
Manager, to perform the functions of Finance
Administrator as defined below in Paragraph A. 2 .
2 . The functions of the Finance Administrator shall
include, but shall not be limited to, the following
duties :
a. Attend City Commission meetings as needed to
inform the Commission on matters involving the
City's financial matters .
b. Plan, organize and review all of the City' s
financial procedures and methods .
C . Prepare monthly financial reports for City
officials .
d. Supervise preparation of the City' s annual
financial statements in cooperation with
independent auditors .
e . Assist in the preparation of the City' s annual
budget.
f . Monitor the City's budget during the fiscal year
and advise the City departments regarding
budgeting problems .
g . Analyze the City' s expenditures , revenues and
investment accounts for accuracy and conduct audit
of other revenue sources .
h. Implement the City ' s central purchasing system.
i . Supervise the preparation by the City of
periodic reports for state, federal and local
authorities as required by law.
j . Provide necessary supervision and
administrative guidance to the City ' s support
staff.
B. COMPENSATION:
Blasi shall provide thirty (30 ) hours of service per
week to the City and shall be compensated $639. 90 (i .e .
$21 . 33 per hour) for such service . If , at the request
of the City and with the approval of the City Manager,
Blasi shall perform more than thirty ( 30 ) hours of
service to the City during any week , the City shall
compensate Blasi for such service on an hourly basis at
the rate of $21. 33 per hour for each hour of service
performed by Blasi in excess of thirty ( 30 ) hours during
any week . Payment shall be made by the City to Blasi on
the basis of a bi-weekly pay period.
C. INSURANCE:
1. The City will cover- Blasi under its Worker ' s
Compensation and General Liability Insurance
policies , including covrage for errors and
omissions, while this Agreement is in full force and
effect and while Blasi is operating within the scope
of services defined in this Agreement .
2. The City shall cover Blasi and her eligible
dependents under the City ' s group health and dental
insurance program. The City shall cover Blasi under
the City's group life and disability insurance
program. Cost for said coverages shall be borne by
the City. Coverages shall become effective November
1 , 1988.
D. RETIREMENT:
In addition to the Compensation payable to Blasi pursuant
to Paragraph B, The City also will deposit an amount
equal to 13. 9% of Blasi 's gross earnings into a deferred
compensation plan to be offered by the City. Blasi may
choose between alternative plans which may be offered by
the City. Contribution on Blasi ' s behalf shall not
exceed the maximum allowed under the Internal Revenue
Code .

E. LEAVE:
Blasi' s obligation to provide 30 hours of service to the
City per week shall be subject to the following provisions
for Leave:
1 . Blasi shall be entitled to sixty (60) hours of holiday
leave per annum which will be earned retrospectively at
the rate of fifteen ( 15 ) hours after completion of each
three ( 3 ) months of service .
2 . Vacation leave will be accrued at the rate of 1 . 75
hours per week and will be earned retrospectively after
completion of six ( 6 ) months of service.
3 . Sick leave will be accrued at the rate of six ( 6 ) hours
per month and will be earned retrospectively after
completion of six (6 ) months of service .
4 . Maximum accrual of Leave is 220 hours . Excess Leave is
converted to cash at 100% of value based on the pay
" rate of $21 . 33 per hour. 100% conversation of all
accrued Leave on separation from the City after
completion of the three ( 3) months of service.
5 . Except as otherwise stated herein, administration of
Leave provisions will be in accordance with the City's
personnel rules, as amended from time to time.
F. CLOSURE:
This Agreement becomes valid upon execution by the
appropriate City Official in the space provided below. This
Agreement shall be in full force and effect for the period
of twelve ( 12 ) months, commencing October 1, 1988, and shall
be deemed renewed for successive periods of twelve ( 12)
months unless cancelled by either Blasi or the City prior to
commencement of any such successive twelve ( 12 ) month
period. This Agreement may be cancelled ( i) by Blasi with a
thirty ( 30) day written notice, or ( ii) by the City with a
ninety ( 90) day written notice . This Agreement may be
modified in writing between the parties from time to time.
