BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY OF DANIA, FLORIDA. Section 1. That the application for the 25`h Year Community Development Block Grant Program between the City of Dania Beach and Broward County, a copy of which is attached hereto as Exhibit "A", is hereby approved and the appropriate city officials are hereby directed to execute same. Section 2. That said application provides for continued sidewalk improvements in the Northwest section of the City involving the College Gardens/Modello Park community not to exceed $300,000.00. Members of both communities met jointly on January 28, 1999, and February 2, 1999. The combined group achieved consensus that sidewalk projects be pursued. Although no consensus was reached on the specific streets where sidewalks would be constructed and repaired, Dania Beach Blvd from 4th Ave west to its terminus, as well as SW 7 Terr., SW 8th St., SW 9 St. and SW 10th St between Phippen-Waiters Rd. and Ely Blvd. are likely to be included in the project. Other streets will be included subject to funding availability. Staff also recommends that we commit to match with a combination of in-kind and cash funds up to 100 percent of each project. As you may know, twenty points are granted by the Broward County Community Development Selection Committee in the application review process. We have had difficulty in the past with getting projects approved because of our inability to provide matching funds for these projects. The provision of matching funds in both applications will aid our chances in what has become an extremely competitive grant process. r �r— 0 ti r -r APPLICATION CHECKLIST 1. Application Submission Deadline: Date-February 15, 1999 Location Community Development Division 201 South Andrews Avenue, Second Floor Fort Lauderdale,Florida 33301 Time-No Later than 5:00 PM, EST All applications received after 5:00 PM will be disqualified Applications submitted for financing of prior debt will not be considered 2. Submit (1) original and (10) copies: Use Only Format(s) Issued by the Community Development Division. 3. All pages must be numbered. 4. Do not insert application founts or formats from prior application or other funding programs. 5. Original Application-do not use staples binders, perforated or bind. 6. Application copies-staple,clip, or use rubber bands. 7. Every page must be on standard sae paper. 8. Answer Each Section In the Order provided. 9. Each Entity must submit a separate application for each proposed activity. 10. Each applicant may submit up to three(3)applications for CDBG funding. Applicants will be disqualified if the total of all requests for funding exceed$300,000 dollars. 11. The minimum project amount is $50,000 dollars but exceptions may be made in instances where CDBG,ESGP,HOME funds leveraged a substantial amount of other funds 12. Applications for CDBG,ESGP,HOME projects located within the entitlement cities of Fort Lauderdale,Sunrise,Lauderhill,Davie,Pompano,Hollywood,and Pembroke Pines will need to serve a majority of clients from the urban county area. 13. All applications must provide certifications, corporate documents, project financial commitment letters to support budget summaries. 14. All acquisition projects must provide an appraisal dated December 1998 or thereafter and evidence of site control. Broward County Community Development Division Request for Proposals 5 Twenty-Fifth Year t- r et r, r .t / • APPLICATION SUBMISSION REQUIREMENTS The applicant must select the application form that meets the eligibility requirements for the application format. The application format is based on Department of Housing and Urban Development (HUD) guidelines for the federal funds under the Community Development Block Grant(CDBG), Emergency Shelter Grants Program (ESGP) and HOME Investment Partnership Program( HOME) funding eligibility, requirements for project activities. As such, the formats described below are based on entity type, targeted income groups, and or income eligible neighborhoods served. Prior to selecting a format foryour project please read carefully the(3) three application formats summarized below. • Please Note Technical assistance upon request should be considered prior to your selecting a format for our project or program. APPLICATION EXHIBIT I- CDBG and ESGP CDBG: The Community Development Block Grant (CDBG) provides funds to serve low and very low income households or neighborhoods. Program funding areas: Public services, capital projects, economic development initiatives and affordable housing projects for existing or new construction single family/multifamily. It should be noted that all applicants structuring housing projects are hereby advised to consult the community development staff thirty(30) days prior to the application submission deadline. Eligible entities:Municipalities,non profit organizations,community based corporations known as Community Development Corporations(CDC). Should your corporation meet the above criteria thresholds select application I(see appendix-Exhibit ESGP: The Emergency Shelters Grants Program(ESGP)provides funds to serve the homeless or at risk homeless population. Program Funding Areas:Facility conversion projects,emergency housing,supportive services,and homeless prevention project activities. Priority shall be given to projects that expand the county's dedicated units for homeless and special needs populations. Eligible Entities: Municipalities,non profit organizations of all types that do not require religious prerequisites for service availability. In the case of religious entities selecting acquisition and or rehabilitation of an existing unit,the property must be owned by a subsidiary nonsectarian entity. Budget Narrative Instructions- The budget narrative statement should provide a detailed explanation justification for each cost category shown in the Budget Table on page 6. The budget narrative should identify non-CDBG/ESG resources to be utilized in financing including mortgages or construction financing for the project. Also, Specify the costs for which CDBG/ESG funding is being requested and the costs to be covered by on-CDBG resources. Additional pages can be added if necessary. Describe and Calculate Leveraging: Describe and attach supporting documentation including letter(s)of commitment,resolutions, minutes of meetings, etc. providing the specific resources the applicant will commit to the project identified in columns 2 and 3 of the Budget Table above. Include and identify in-kind contributions, sweat equity and other resources. a_ Documentation must be provided as an attachment and summarized below. 1) For economic development projects,the value of the jobs produced in the previous year can be used as leverage.showing name of business, title of job and salary. 2) For housing development projects,anticipated mortgage financing will be considered as in-kind contribution. b. Instructions on Calculating Leveraging Leveraging will be computed by taking into account the total dollar cost of the entire project including in-kind contribution. Percent Leveraging=Total Non-Grant Funds(CDBG/ESG) $X 100 Total CDBG/ESG$ In computing Total Cost of the project,funding from all sources for the project must be added. This includes Federal funds, State funds, Municipal funds, contributions, private sector financing,in-kind contributions,etc. In-Kind Contributions are non-cash items. Non-cash items are contributions to the project, a g., labor, office space, use of equipment, etc., that do not involve cash payments by the entity. However,a dollar value must be given to in-kind and it must be added to the cost of the project. In-kind contributions must be accounted for and included in the financial audit of the funded entity.
