A RESOLUTION OF THE CITY OF DANIA, FLORIDA,
APPROVING AN AMENDMENT TO THE EMPLOYEE HEALTH
BENEFIT PLAN AND SCHEDULE OF PARTIES TO THE
PLAN AS APPROVED BY RESOLUTION NO. 22-86 ON
MARCH 25 , 1986 ; AND PROVIDING THAT ALL
RESOLUTIONS OR PARTS OF RESOLUTIONS IN CONFLICT
HEREWITH BE REPEALED TO THE EXTENT OF SUCH
CONFLICT; AND PROVIDING FOR AN EFFECTIVE DATE.
BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY OF DANIA, FLORIDA :
Section 1 . That the Employee Health Benefit Plan and Schedule
of Parties to the Plan as approved by Resolution No. 22-86 on March
25, 1986 , and amended by Resolution No. 52-86 on Octo
ber 14, 1986, be
hereby further amended by Plan Amendment No. 6.

Section 2. That said Plan Amendment No. 6, incorporated herein
and made a part hereof by reference, be and the same is hereby

i approved and the appropriate City Official is hereby authorized and
{9J9 directed to execute same on behalf of. the City of Dania , Florida.
I Section 3. That said Plan Amendments and the original of said l w-t
S Employee Health Benefit Plan and the original of Schedule of Parties

to the Plan shall be kept in the office of the City Clerk-Auditor.
No changes or alterations shall be made to any of said documents

without the approval of the City Commission .
Section 4 . That all resolutions or parts of resolutions in

conflict herewith be and the same are hereby repealed to the extent
of such conflict .
Section 5. That this resolution shall be in force and take
effect immediately upon its passage and adoption.

PLAN AMENDMENT NO. 6
Group Plan Number: 501 - 20928
Employer : City of Dania, Florida
Notwithstanding any language in the Plan to the contrary, effective
October 1, 1986, it is hereby understood and agreed that the above
Group Plan Document shall be amended as follows :
MENTAL & NERVOUS CONDITIONS
In accordance with Florida State Law, the Plan shall
pay benefits for Mental & Nervous conditions, up to the
minimum requirements of the Statute, in the manner
shown below:
Out-Patient (Per Visit ) 80%
Calendar Year Maximum (Out-Patient) $1, 000.
In-Patient Services 80%
Calendar Year Maximum (In-Patient) 30 Days
" All eligible Mental & Nervous charges shall be subject
to the Major Medical Deductible. However, such charges
are not eligible for benefit reimbursement under the w ,
"Full Payment Feature" of the Plan.

This amendment is hereby incorporated Into and made a part of the
said Group Plan Document, however, except as expressly stated above,
this Amendment does not waive, alter or extend any provisions or
limitations of the Plan.
