A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF DANIA
BEACH, FLORIDA, AUTHORIZING THE PROPER CITY OFFICIALS TO
EXECUTE CHANGE ORDER NOS. 1 AND 2, AND TO PROVIDE FUNDING
FOR ADDITIONAL CAPITAL PROJECT SERVICES WITH CENTURY
BUILDING RESTORATION USA INC., FOR THE I.T. PARKER DOCK
REPLACEMENT PROJECT IN THE AMOUNT NOT TO EXCEED $6,750.00;
PROVIDING FOR CONFLICTS; FURTHER, PROVIDING FOR AN
EFFECTIVE DATE.
WHEREAS, the City awarded a contract on April 10, 2012, to Century Building
Restoration USA Inc. for the I.T. Parker Dock Replacement Project; and
WHEREAS, a contract between the City of Dania Beach and Century Building
Restoration USA Inc. was executed for the Project in the amount of$54,260.00; and
WHEREAS, structural cracks to the existing pile caps were discovered by the Contractor
and two (2) Change Orders were necessary for a total cost of$6,750.00;
NOW,THEREFORE,BE IT RESOVED BY THE CITY COMMISSION OF THE
CITY OF DANIA BEACH,FLORIDA:
Section 1. That the foregoing "Whereas" clauses are made a part of and incorporated
into this Resolution by this reference.
Section 2. That the City Commission approves and City officials are authorized to
execute Change Order Nos. 1 and 2 in the total sum of$6,750.00; copies of the Change Orders
are attached as Exhibits "A" and "B", which are made a part of and incorporated into this
Resolution by this reference, for the additional Capital Project services for the pile cap repair
with Century Building Restoration USA Inc. on the I.T. Parker Dock Replacement Project.
Section 3. That the cost of these Change Orders shall be transferred to the Capital
Project Fund — I.T. Parker Dock Project from available budget appropriations in the General
Fund-Parks&Recreation, Professional Services Account, Account No. 001-72-01-572-31-10.
Section 4. That all resolutions or parts of resolutions in conflict with this Resolution
are repealed to the extent of such conflict.
Section 5. That this Resolution shall be in force and take effect immediately upon its
passage and adoption.

1- Pile cap under the exisitng dock has several areas where we believe there to be structural
Cracks in need of repair there are 8 locations currently marked with paint totlaing 40 linear
Feet which will all require partial depth edge repair one area will require the repalcement of reabar
see attached photos 3 pages Edge repair $ 2,900.00
Rebar replacemen $ 75.00
2- Pile#3 and# 17 counting from East to West have what is called Brown Eyes rebar is to close
to the surface rusting and bleeding through in one location
Brown eyes $ 225.00
TOTAL $ 3,200.00
Not valid until signed by both the Owner and Engineer.
Signature of the Contractor indicates his agreement herewith including any adjustment in the Contract Sum or Contract Time.
