A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF DANIA
BEACH, FLORIDA, AUTHORIZING THE PROPER CITY OFFICIALS TO
RENEW AN AGREEMENT WITH MAXON ADMINISTRATORS, INC. TO
PROVIDE CLAIMS ADMINISTRATION FOR A SELF-FUNDED DENTAL,
VISION AND DISABILITY PLAN TO BE OFFERED TO ELIGIBLE
EMPLOYEES AND RETIREES, AND TO INCLUDE IN THE AGREEMENT
ACCESS TO A DENTAL PREFERRED PROVIDER ORGANIZATION
NETWORK IN AN AMOUNT NOT TO EXCEED $9,800.00 FOR FISCAL
YEAR 2012-2013; PROVIDING FOR CONFLICTS; FURTHER, PROVIDING
FOR AN EFFECTIVE DATE.
BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY OF DANIA
BEACH,FLORIDA:
Section 1. That the City Commission waives bidding for dental, vision and disability
administration services and authorizes the proper City officials to execute documents pertaining
to the renewal of an agreement with Maxon Administrators, Inc. ("Maxon") to provide third
party claims administration services to the City of Dania Beach, Florida for a self-funded dental,
vision and disability plan to eligible employees, retirees or both, at a cost not to exceed $3.75 per
employee or retiree per month.
Section 2. That Maxon is authorized to administer a Preferred Provider Organization
option for.the dental plan, and the cost of such administration will be broken down as follows:
access to the DHA network at a cost not to exceed $0.65 per employee or retiree or both, per
month, a Maxon re-pricing administration fee not to exceed $0.35 per employee or retiree or
both,per month, and a$1.00 cost per I.D. card per employee,retiree or both.
Section 3. That funding for the cost of the City's dental claims administration
program shall be charged to the Health Insurance Fund, Professional Services Account, Account
No. 5 01-18 00-519-31-10.
Section 4. That based on recommendations of the City actuary, the City Commission
approves and adopts the dental, vision and disability self-insured plan administered by Maxon
Administrators, Inc. for the 2012-2013 fiscal year at a monthly premium of $80.41 for single
coverage and $213.11 for family coverage.
Section 5. That all resolutions or parts of resolutions in conflict with this Resolution
are repealed to the extent of such conflict.
Section 6. That this Resolution shall be in force and take effect on October 1, 2012.
