Dear Ms. Pollard:
This letter serves as invoice #1 for the above project. The tasks are as follows:
Task 1 – SE 5th AND SE 11th TERRACE LIGHT REQUEST - $1,000.00
Public Utility Management and Planning Services, Inc. contact Broward County Transportation Division
to assess how to request light, and develop documents to support the request. The cost for this work will
be $1,000.00.
Status: Complete
Task 2 - SE 5th AND SE 11th CROSSWALK AND LIGHT INSTALLATION PACKAGE -
$5,000.00
Public Utility Management and Planning Services, Inc. will a create a design package for a crosswalk and
push button light for bidding and installation.
• Removal of certain existing pavement
• Installation of brick crosswalk
• Installation of striping
• Installation of two poles and traffic light
Said bid package will be submitted to the City for review and approval, including interaction with the
City Attorney’s office as needed. Installation will be in accordance with the attached, in accordance with
Broward County standards. Several existing speed humps may be removed and not replaced. The cost
for this work will be $5,000.00.

Task 3 – SE 5th AND SE 11th CROSSWALK AND LIGHT INSTALLATION Bidding and Bid
Recommendation –$2,000.00
Public Utility Management and Planning Services, Inc. will coordinate bidding, a pre-bid meeting,
develop addenda as needed to answer all questions, review the bids, provide a recommendation to the
City for the Bid and provide help to the City in getting the information necessary to develop the executed
contract documents. Public Utility Management and Planning Services, Inc. will coordinate the second
phase of the project at a cost of $2,000.00 for the bidding process (through recommendation). Attendance
at the City Commission meeting is not required.

Task 4 – SE 5th AND SE 11th CROSSWALK AND LIGHT INSTALLATION PACKAGE -
Construction Project Oversight - $2,000.00
Public Utility Management and Planning Services, Inc. will perform the following tasks for the City for:
• Preconstruction conference
• Monthly progress meeting
• Review of project schedules
• Development and review of monthly invoices
• Recommendation for payment of monthly invoices
• Site visits
• Response to RFIs and phone calls
• Review of shop drawings
• Contractor coordination
• Review of materials
• Necessary Communication to CDBG program
• Submittal of monthly Exhibit D reports
The billing from Public Utility Management and Planning Services, Inc. will be a total $2,000.00.

Task 5 – OLD GRIFFIN ROAD LIGHTING –$1,000.00
Public Utility Management and Planning Services, Inc. will review the City’s current speed hump policy
and make recommendations for alterations to same. In addition, Public Utility Management and Planning
Services, Inc. develop a standard for speed hump installations in the City, including distances apart,
signage etc. Said standards will be based to the extent possible on existing standards in Broward County
and other local communities. The cost for this work will be $1,000.00.

Task 6 – SITE PLAN REVIEW SARATOGA –$1,000.00

Task 7 – CIP CONSULTATION –$500.00
Public Utility Management and Planning Services, Inc. will review the City’s current speed hump policy
and make recommendations for alterations to same. In addition, Public Utility Management and Planning
Services, Inc. develop a standard for speed hump installations in the City, including distances apart,
signage etc. Said standards will be based to the extent possible on existing standards in Broward County
and other local communities. The cost for this work will be $500.00.

Task 8 – UTILITY AVAILABILITY W. DANIA BEACH BLVD –$500.00
Public Utility Management and Planning Services, Inc. will review the water and sewer availability for
the 70 Wave, 90 Wave and 150 Bryan Rd projects and provide draft letters for the City staff for review,
before providing to the developers. The cost for this work will be $500.00.
Status: Complete
TOTAL WORK ORDER - $13,000.00
Please note I do not see Oasis XIV on this work order so maybe there is some confusion here? Please
advise, but in the meantime a lot of this work s done.
