Please confirm the refund of $48.93 was issued to Mr. Green, as requested below. Also advise on the result of your reconciliation from 20/21, as we may owe him additional money. Please provide your calculations. Please find the attached spreadsheets for Jack Green 2020/2021 reconciliation calculations based on the checks received and posted by finance. Mr. Green is owed $193.95 plus the $48.93 you have listed in overpayment amount. A check request will be submitted to finance to be processed in the amount of $242.88. 2. Please confirm the check was received in the amount of $528.16 for Dorothy Cook. Dorothy’s assistant dropped off a check in the amount of $628.12 to cover the amount owed and January premiums. Please see receipt attached. 3. Please confirm letters were sent to retirees advising them of the new premium amounts due by 1/1/22 to Mr. Cook, Ms. Green and Mr. Kirby. Jack Green - Please do a check request for $48.93 for the last 3 months that Jack Green has overpaid premiums. His monthly bill for Oct, Nov, Dec 2021 is $144.58. He paid $160.89. Here is the breakdown that you can include in the letter: paid $1,642.59 for FY 20/21, then she owes the City the difference of $94.42. Medicare Advantage (10/1/20 - 12/31/20) = $411.43 (3*$137.14) Medicare Advantage (1/1/21 - 9/30/21) = $1,199.70 (9*$133.30) Dental (10/1/20 - 9/30/21) = $105.24 ($8.77*12) Vision (10/1/20 - 9/30/21) = $20.64 ($1.72*12) Total: $1,737 Additionally, Dorothy has not paid her premiums for Oct, Nov and December 2021, and they must be paid immediately. Dorothy owes the City $133.30/month for medicare for three months plus, plus $9.82/month for dental for 3 months + $1.46/month for vision for 3 months = $433.74 Please confirm these amounts are correct. The total due the City for 2020 and 2021 is: $528.16. Please advise Dorothy to submit the check for the past due amount no later than Monday, January 3.
